National Disability Insurance Scheme (NDIS)

Dear Parents/Guardians,

As you may be aware, the National Disability Insurance Scheme (NDIS) is currently being rolled out in Victoria. More specifically, it will be rolled out to the Inner Eastern region of Melbourne from November 12017.  The aim of this scheme is to provide fairer and individualised support to assist individuals achieve goals and become more independent.

If you live in the City of Monash (Hughesdale, Oakleigh, Huntingdale, Oakleigh South, Oakleigh East, Clayton, Notting Hill, Mount Waverley, Glen Waverley, Wheelers Hill, Mulgrave, or Chadstone) you will be able to apply for the NDIS by contacting your Local Area Coordinator (LAC) at the Latrobe Community Health Service on 1300 242 696. If you live in another area, please refer to the links below which will guide you as to who you need to contact.

Please be aware that weare informing you of this service, which you may decide to apply for on behalf of your son. If you are currently accessing services, you should be contacted by a representative to discuss the process of entering the NDIS. If you are not currently receiving support, please check the information and resources available at NDIS Victoria in order to assist you with applying. Please refer to the Eligibility Criteria and the supporting documentation.
